• You wish to report that you have completed your prescribed practical experience requirements
• You wish to apply for admission to the Ontario CA membership

When completing the form, please bear mind that any leaves of absence that are taken in excess of the allowable leaves will affect the completion of the prescribed practical experience requirement.

When your experience has been fully documented, we will be sending you a personalized Admission to Membership form. Please return the completed form, along with your payment of the Admission fee of $565.00 ($500.00 plus HST) to have your membership review by the Institute.
