2013 Selection of Offices for Practice Inspection
ACCESSING THE PLANNING QUESTIONNAIRE
The planning questionnaire is accessed through the Planning Questionaire Site Login.
Access to the questionnaire is restricted to the Practice Inspection Contact, the addressee of your office’s selection letter. To change the Practice Inspection Contact, one of the firm’s designated contacts must log in to “Services for CA Firms and CATOs” and complete the “CA Firms/CATO Address and Contact Information Updates” (refer to FAQ #2 for further details, including a definition of the firm's "designated contacts"). NOTE: If the Practice Inspection Contact and/or firm's address changes AFTER submission of the questionnaire, please contact the coordinator named in your selection letter.
You must log in using your ICAO Member # and password.
For firms with multiple offices selected for inspection, a separate planning questionnaire must be completed for each office, i.e. for each inspection number. Accordingly, information will need to be determined (or approximated for data with respect to number of engagements and engagement hours) for each office.
Caution! - You must log off and log back in to start/continue/complete each office's questionnaire.
After logging in, you will be asked to read an overview page. You will then be presented with a list of four possible planning questionnaires. Select the one that best describes your office’s current practice.
COMPLETING THE PLANNING QUESTIONNAIRE
The planning questionnaire is organized by tabs. Each tab includes a series of questions relating to your office’s practice.
You can navigate between the tabs using the “next tab” and “previous tab” buttons at the bottom of the page or by clicking on the respective tabs at the top.
Reminder! – Save periodically.
As you complete the questionnaire, ensure to save periodically by clicking the “save” button at the top. If you expect to be away from the questionnaire for any extended period of time, please save and log off entirely as the session will expire. You can return to any planning questionnaire that has been started and saved in a prior session, but not yet submitted.
PREVIEWING AND SUBMITTING THE PLANNING QUESTIONNAIRE
Once you have completed the questionnaire, you may preview your responses prior to submitting. Click the “Save & Preview” button at the top. You may “preview” as often as you like and at any point during the completion of the questionnaire, and return to the questionnaire to make any amendments.
When you are satisfied that the questionnaire is complete and ready to submit, click the “Submit Questionnaire” button at the top.
Caution! – You will not be able to make any changes once you have submitted the questionnaire.
You will receive a confirmation number as evidence that the questionnaire was successfully submitted. There will also be an option to receive an email (highly recommended) of the confirmation number with a link to your summary page. You may also print a copy of this page for your records.
Practice inspection will assign an inspection date and an inspector based on the information submitted. A letter will be sent to the individual identified as the practice inspection contact in the questionnaire at least 30 days prior to the assigned inspection visit date (21 days for a desk inspection).
